Risk-Based thinking
We are likely familiar with the study "To Err is Human" from the Institute of Medicine in the late 1990's that states that approximately 98,000 Americans die each year from preventable medical errors. A more recent study published by John T. James, Ph.D. in 2013, using data from 2008-2011, states that "the true number of premature deaths associated with preventable errors was estimated at more than 400,000 per year". His conclusion states "The epidemic of patient harm in hospitals must be taken more seriously if it is to be curtailed. Fully engaging patients and their advocates during hospital care, systematically seeking the patients’ voice in identifying harms, transparent accountability for harm, and intentional correction of root causes of harm will be necessary to accomplish this goal." Unfortunately, accreditation data supports these new findings. This revelation is after we have created a "safety culture", added staff and oftentimes a new department for Risk or Patient Safety or Quality and greatly increased our awareness on these issues.
